We Handle the Insurance Details for You Our team will begin by collecting your insurance claim number and details about your case. We’ll then contact your insurance adjuster on your behalf to verify your Personal Injury Protection (PIP) or MedPay coverage and confirm your eligibility for massage therapy. If a doctor’s referral is required, we’ll walk you through the process of obtaining the necessary prescription or medical note that includes: ICD-10 diagnosis codes Specific areas to be treated (left/right, upper/lower body) Frequency and duration of treatment Recommended type of massage therapy Timeframe for treatment sessions Referring provider’s name, signature, and date What to Bring to Your Appointment: ✔ Referral or prescription (if required by your insurer) ✔ Insurance claim number or insurance card ✔ Any diagnostic imaging (e.g., MRI, X-rays) ✔ Comfortable clothing to allow access to treatment areas Your First Visit: What to Expect Your initial session will include a detailed intake and clinical assessment to understand the full extent of your injury. Cancellation Policy
CANCELLATION POLICY We understand that unanticipated events happen occasionally in everyone’s life. Business meetings, project deadlines, flight delays, car problems, snowstorms, and illness are just a few reasons why one might consider canceling an appointment. In our desire to be effective and fair to all of our clients and out of consideration for our therapists’ time, we have adopted the following policies: • 48 hour advance notice is required when cancelling an appointment. This allows the opportunity for someone else to schedule an appointment. If you are unable to give us 48 hours advance notice you will be charged the full amount of your appointment. This amount must be paid prior to your next scheduled appointment. • No-shows: Anyone who either forgets or consciously chooses to forgo their appointment for whatever reason will be considered a “no-show”. They will be charged $45 for their “missed” appointment and future service will be denied until payment is made. • Arriving late Appointment times have been arranged specifically for you. If you arrive late your session may be shortened in order to accommodate others whose appointments follow yours. Depending upon how late you arrive, your therapist will then determine if there is enough time remaining to start a treatment. Regardless of the length of the treatment actually given, you will be responsible for the “full” session.
